I, the client (18 and above) agree to participate in therapy and give consent for my therapist to disclose & receive information/reports to and from my emergency contact and my referrer, as well as other supports I give consent for.
I also acknowledge that I have read and agree to my rights and responsibilities in relation to therapy and regarding the Therapy with Taf and consent to participate.
As a client you have the following rights and responsibilities:
Privacy and Confidentiality
As part of providing therapeutic counselling to you, Therapy with Taf will need to collect and record personal information about you that is relevant to your current situation. This information will be a necessary part of the psychosocial assessment and treatment plan. Your information will be kept strictly confidential and will not be disclosed without your consent. However, there are certain exceptional circumstances where legal or professional rules may require disclosure of your information.
These situations include:
1. If your information is subpoenaed by a court
2. If your therapist determines there is risk that you may harm yourself or another person including a minor
3. If your prior approval has been obtained to a provide a written report to another professional or agency, e.g., a GP or a lawyer or to discuss the material with another service or person. e.g., GP or employer.
PLEASE NOTE: If any of these situations arise, I will endeavour to discuss with you prior to disclosing any information.
Emergency Situation
Triple (000), medical treatment or an ambulance will be sort in emergency situations. Therapy with Taf takes no responsibility for the cost of this medical treatment.
Support Services Offered
Respect will be shown to you and your background, sexual orientation, identity, culture or ethnicity.
You will receive a clear explanation of the service you will receive.
Your therapist will be clear and transparent regarding any assessment of your priorities and counselling needs.
You will receive an explanation about confidentiality and its limits (including risk issues and subpoenas).
Your Therapist may need to discuss your situation with other professionals during supervision (or consultation), in order to best meet your needs. In these situations identifying information will not be provided in order to safeguard your identity.
Your therapist will let you know as soon as possible of any cancellation or changes to your appointment due to the illness or unavailability of your therapist.
You are free to terminate your service at any time.
Payment
The full fee for an individual counselling session is $160 Inc GST, professional supervision is $160 Inc GST. Payment is required in full after each session. This is an affordable cost which takes into consideration no availability of any rebates.Therapy with Taf accepts payment via Square (all major credit/debit cards accepted), direct deposit into bank account or card payment or an invoice can be sent to you via email to be paid within TWO DAYS after your session. Alternatively, you may choose to do a banking transfer to a nominated account and the details with be provided.
Reports
Any reports are charged from $180 including GST, depending on the length and time required to complete the report.
NDIS Funding
The cost per hour for NDIS counselling is billed as per NDIS Pricing Arrangement.
The amount of sessions will be negotiated and set, depending on your Plan and the availability of your therapist.
Approval of using your NDIS Plan for this service will need to be provided prior to your first session.
There will be no additional costs, other than that originally agreed upon.
If further services/fees are required these will be negotiated prior to their commencement.
This agreement includes one review letter for your NDIS review per year, but no other reports.
The amount of sessions will be negotiated and set, depending on your Plan.
As a non-registered provider, your therapist cannot provide a service if you are NDIA managed. Only plan and self managed plans are accepted.
Rescheduling/Cancellation
If for some reason you need to cancel or reschedule, please give sufficient notice otherwise you will be charged 50% of the fee if less than 48 hrs is provided and the full fee if less than 24 hrs notice is provided.
